About the Author

Thomas G. Gunning has taught courses in methods of teaching reading and writing for more than 20 years and was director of the Reading Clinic at Southern Connecticut State University. Before that, as a secondary English teacher, a reading specialist, and an elementary school reading consultant, he worked extensively with achieving and struggling readers and writers. Dr. Gunning, who recently served as a Reading First consultant, is currently working with elementary and middle school students to help them develop higher level literacy skills, as well as serving as an adjunct professor in the Reading/Language Arts Department at Central Connecticut State University. Over the years, Dr. Gunning’s research has explored reading interests, informal reading inventories, decoding strategies, readability, higher-level literacy skills, and response to intervention. As a result of this research, he has created a number of informal assessments and programs for developing decoding and comprehension skills.

“Quality of Life” and “Health Outcomes” and other terms referring to the physical, mental and emotional effects experienced by people as they encounter health challenges are in wide use and misuse in the health care and health research community. Many of these terms are used interchangeably without recognizing that they have completely different meanings and, thus, require different measurement approaches and different types of interventions if they are to be optimized. The notion of creating a “dictionary” of these terms arose so that novices and experts, researchers and clinicians, can commonly address these health outcome challenges. The correct term for this collection of definitions is a “vocabulary for a vertical audience” as the definitions reflect the usage in QOL and health outcomes measurement rather than all usages and the definitions were written to have meaning for the novice while still being useful for the expert

As a starting point for the vocabulary, terms were chosen based on the frequency of use in journal articles focussing on of quality of life research. The terms fall broadly into 8 concepts all of which have an identified need for consistent and correct terminology: research process, measurement properties, statistics for QOL research, designs for QOL research, patient reported outcomes (PROs), theories and models, knowledge translation, and personal factors. Rather than being “A malevolent literary device for cramping the growth of a language and making it hard and inelastic” (Ambrose Bierce, The Devil’s Dictionary), “This dictionary, however, is a most useful work.” (Ambrose Bierce, The Devil’s Dictionary).

To avoid the “devil”, terms reflecting modern advances in outcomes research and design were included even they may not yet be in common usage in QOL research. For example, while designs like the stepped wedge or platform trials may not yet have penetrated the world of QOL research, they will not if no one knows about them. More than 20 people contributed definitions and following editing, the dictionary was opened up for review and comment from members of ISOQOL and their suggestions and modifications were included, adding a component of peer review to the process.

Nancy E. Mayo, PhD Nancy Mayo is a Professor at McGill University in the Department of Medicine, Division of Clinical Epidemiology, and School of Physical and Occupational Therapy. Born in Ottawa, Ontario, Canada, she trained as a Physical Therapist at Queen’s University, and after several years of working clinically, she completed a doctoral degree in Epidemiology and Biostatistics, at McGill University. She has been educating health professionals and clinical researchers for over 25 years and has published over 200 scientific works in the peer reviewed medical literature as well as contributing to collective works such as Medical Uses of Statistics 4th Edition (Ed. Bailar and Hoaglin) and Physical Rehabilitation Outcome Measures (Finch, Brooks, Stafford, Mayo). Mayo has also written a number of scientific papers specifically related to education on research methods and statistics. The Dictionary of Quality of Life and Health Outcomes Measurement was inspired by students and was assembled as a way of helping novices and experts alike to “speak the same language” about measuring these important health outcomes; essentially, the intent was to improve the research-related quality of life of people working in this important field.

"This day a herd of twenty-eight giraffes tantalized me by descending a short distance below the level flats, and I was tempted at all hazards across the river. Accordingly preparations were immediately made for a start. "This being a concise summary of the Nile system, I shall describe twelve months' exploration, during which I examined every individual river that is tributary to the Nile from Abyssinia, including the Atbara, Settite, Royan, Salaam, Angrab, Rahad, Dinder, and the Blue Nile."
Sir Samuel Baker, who had spent a number of years in Ceylon in the mid-19th century was eager to explore new territories. The source of the Nile was still a great mystery in the 1800's. European speculation about Africa had long existed: ancient authors like Herodotus and Strabo wrote about it, and British schoolboys grew up wildly excited and eager to go there from reading these authors. David Livingstone, Richard Burton, John Speke, and Samuel Baker, among others, would base their expeditions partially upon these ancient texts. (Now aren't you sorry that you didn't learn Latin?)
Baker's earlier travels through Africa had led him to the lake that he named after Prince Albert. Now he was back in Africa to continue not only his exploration of the Nile and its tributaries, but also for ethnographic research, and hunting. Well, actually for hunting along the Nile tributaries.
He spent many hours with the Arabs of the region, and the book is full of his trenchant observations about their customs. He was especially thrilled when he found the Harman sword hunters, for he had heard of their skill in elephant hunting by sword alone. Here is his description of that important weapon:

"The Arabs are extremely proud of a good sword, and a blade of great value is carefully handed down through many generations. The sheiks and principal people wear silver-hilted swords. The scabbards are usually formed of two thin strips of elastic but soft wood, covered with leather. No Arab would accept a metal scabbard, as it would destroy the keen edge of his weapon. The greatest care is taken in sharpening the swords. While on the march, the Arab carries his weapon slung on the pommel of his saddle, from which it passes beneath his thigh. There are two projecting pieces of leather, about twelve inches apart, upon the scabbard, between which the thigh of the horseman fits, and thus prevents the sword from slipping from its place. Carried in this position at full speed, there is an absence of that absurd dangling and jumping of the sword that is exhibited in our British cavalry, and the weapon seems to form a portion of the rider."
Baker crossed deserts littered with camel skeletons, camped along riverbeds near hippopotami, and hunted the numerous animals of the Nile basin. From desert nomads he learned how Arab women perfume themselves, and from conversation with sheiks he learned the histories of the various tribes. And oh for the resourcefulness of the traveler:

"This day a herd of twenty-eight giraffes tantalized me by descending a short distance below the level flats, and I was tempted at all hazards across the river. Accordingly preparations were immediately made for a start. The sheik of the village and several of the Arabs were hippopotami hunters by profession; these fellows could swim like otters, and, despite the crocodiles, they seemed as much at home in the water as on land. We prepared an impromptu raft. My angarep (bedstead) was quickly inverted; six water-skins were inflated, and lashed, three on either side. Baker does describe the Nile tributaries but the majority of his time seems to be taken up with his hunting: wild game and people are just more interesting to him. Yes, he can be pompous, but the world has shrunk since Baker's day, and his book captures all the excitement of being in new territory. And if you also need to know how to ford a swift river with a horse, you will find the technique in chapter 15.

Early African Adventure as Only a Brit Could Tell It

You've got to like the sometimes tedious, journal-style approach of early explorers, or at least be willing to put up with it, to appreciate this book. However, it holds some remarkable writing, insight and yes adventure. Baker was a contemporary of Richard Burton, John Henning Speke, Henry Morton Stanley and David Livingstone, all famous for their searches for the source of the Nile River in Africa. If you know anything about this time in history and about Africa itself, then you know that Africa was almost always fatal. Few who went into the interior returned, and those who did usually paid with their health. Not a pretty picture. Into this scene walks Samuel Baker, a wealthy, larger than life British aristocrat with a passion for exploration and excitement.
Baker was a man who tackled rather than simply lived life. A fine linguist, writer, artist and sportsman who hunted with a knife because it was too easy to kill with a gun, he was also extremely practical with a "let's get on with it" attitude towards his travels. Nothing but determined, he presses forward when others would have said, "enough." Are you surprised that he succeeds where others have failed? I wasn't.
What I like about Baker's writing is that he knows how to tell a story. He is as interested in the people as he is in the facts of what happened. It also helps that Baker himself edited this book, picking and choosing the entries from his longer journal. If Baker is pompous and full of his own superiority at times, he can be forgiven. All in all this is a good read. It helps, however to have read his first book "The Albert N'yanza Great Basic of the Nile" first.

One of the greatest adventures of all time

I like to read about exploring and hunting in Africa, authors like Ruark, Capstick, & Hemingway. The reason I hunted down this book is because Hemingway said it was the best book he ever read about Africa. I figured he would know.

I enjoyed this book immensely. Baker describes his explorations, hunting expeditions, and encounters with the natives in experiences that now can only be dreamed about. He talks about his extensive hunting trips with the Hamram Arabs, who hunted elephants with swords from horseback. He tells about traveling hundreds of miles on the backs of camels, and about rescuing one pack camel that had fallen off the side of a cliff by wrapping ropes around it and lowering it to the bottom of the canyon. Even much of the day to day camp life is very interesting, as the party had to provide for all of their needs for themselves.

It is worth noting, that apparently modern readers must be somewhat put off by a supposed tone of superiority. One reviewer indicates this way, and in the version I have, the publisher actually attempts a vague apology on the back of the book. I didn't notice anything in Sir Samuel Baker's tone, but if someone can read this book and not conclude from the narrative that Baker actually is superior to the vast majority of mortals, they must have a better imagination than I do.

One of the most interesting aspects of Baker's explorations is that he had his wife, Lady Florence, with him the entire time. One of my favorite stories is about a night when he was awakened by a gentle tug on his sleeve, which was always his wife's signal that something was wrong. Without rolling over he reached under his cot and got his rifle first, and then his wife indicated that a hyena had just stuck it's head in the door of the tent. He waited a few minutes, and when the hyena took another look, he dispatched it there in the tent in the middle of the night.

If you like to read about Africa, or about exploration, hunting, or high adventure, I think you will enjoy this book very much.

Samuel Baker - some character

This book makes fascinating reading for anyone interested in 19th century Africa. Above all, it gives a clear insight into how Africa and, in particular, the Nile was viewed by European adventurers and hunters at the time. Samuel Baker was a very able writer and his fluid style and keen eye for scientific detail make this book a pleasure to read. Perhaps the only negative aspect of the book is that it goes into too much detail on Baker's hunting expeditions and the modern reader may find some of these episodes quite savage and cruel. Apart from that, it is a book which is well worth reading and which gives the reader a much better understanding of how life really was in these remote areas in the late 19th century.

Awesome Book!!!!

The book was in good condition. There was a little bit of writing in the book, but nothing big and pencil was used. It's a great book. I needed it for a class, but I've recommended it to people that have a hard time choosing a career, need help with resumes and finding a job.

The most valuable career-development tool around!

I had taken a number of career definition tests in my career as an employment counselor (trying to figure out what was my true calling and how I could earn more money). Then I did the work in Michelozzi's book and came up with the scoring that indicated I would be good at counseling/social work (which I was already doing) or contract negotiations. I didn't even know what a contract negotiator was!
I discussed this outcome with my friend who worked at a large corporation. She introduced me to the head of their contracts department. I interviewed him and his staff about the work they were doing. They happened to have a trainee position open (which paid more than what I was getting as a counselor). Now, eighteen years later, I can truly say that Betty Michelozzi's book, Coming Alive from Nine to Five, is the one book which has completely turned my life around. I have loved almost every day of my changed career and I am far better paid at this work than I would e! ver have been as a counselor.
I recommend this book to all of my acquaintances who suspect it is time to change careers--and I recommend it to you. Thank you, Betty, for this truly wonderful opus!

Good for right and left brained...

This book is a well balanced text for all career searchers. It is filled with solid information, great exercises, humor, practical steps, and hope. I use it as a primary text for the Career Exploration, [Community College] class that I teach. It is also used at other institutions.
The text is currently in revision which will make it a 5 Star Text. I would recommend you purchase the new 7th edition, to be released this summer.

Robert W. Kellemen, through his three pastoral ministries, seminary teaching, and RPM Ministries, has equipped hundreds of people as biblical counselors and spiritual friends. Bob is executive director of the Biblical Counseling Coalition and executive director of the Center for Church Equipping at the Association for Biblical Counselors.

Excellent

Kellemen writes an approachable and gospel rich book addressing sexual abuse. Kellemen first makes an overarching statement for everything he'll say--"[God] is Indispensable" (p. 7) and later, "So Satan loves to feed our distrust of God with betrayal by those who ought to be trustworthy" (p. 12). For those who have been sexually abused, he's established the truth and the lie.

These foundations are explored through the story of Amnon and Tamar from 2 Samuel 13 paralleled with the counseling session of "Ashley" and "Nate." We see the biblical narrative unfold and the growth of the gospel in the marriage of Ashley and Nate.

I love the biblical exposition. Pastors, I would encourage you to read this book with an eye to making your sermons more pastoral. Having an eye to applying the gospel as a counselor. Kellemen will demonstrate how this can be done well.

Those who are fighting despair, anger, or other emotions because of sexual abuse, I would encourage you to read with an open heart to God's Word and find a biblical counselor you can trust to lead you to Jesus Christ for healing.

Those who are reading this book because a friend or family member has experienced sexual abuse and you're unsure how to respond, Kellemen provides a good reminder,
One of the temptations we have as Bible-believing Chris- tians is to race in and tell God's story before we've taken the time to listen to our suffering friend's story. We earn the right to interact about God's eternal story by first listening to our friend's earthly story. (p. 22)
No matter who you are remember, "[T]hrough Christ we are accepted in the beloved" (p. 17).

Best Ever

This is by far the best book on this topic I have read. Kelleman gets straight to the point without drawing out painful details (as hearing stories of the affliction of others can possibly further traumatize the traumatized). A must first read for counselors, victims (along with a trusted listening ear of course) and spouse/friend/family of the victim.

As a victor and a Biblical counselor, this book was gentle and compassionate without compromising the Gospel. Kelleman addresses the seriousness of assault and it's damaging effects on the human soul. He points us to The One who can heal and redeem.
One thing we do not want to admit is that we can respond wrongly in response to being hurt. We are prone to, as Paul Tripp so astutely puts it, suffer the way we suffer the thing we are suffering. We are all sufferers and sinners. Kelleman addresses first the sufferer and then delicately addresses the sinner. He again points to The One who forgives and redeems.

Kelleman nailed every point I have pondered in my 8 years of pursuing healing in a goodly manner. I am looking forward to implementing this into my ministry as well as reading it again with my husband.

Finding Hope in Christ by John Henderson (part of the Gospel for Real Life series) is also good.

I would also recommend Rid of My Disgrace as a follow up read but I wouldn't give it to someone who has just been, or just beginning to deal with past assault. Personally it was too big of an undertaking and too rough for me to begin with, though Biblically solid. Perfect for a Biblical counselor or further study for the spouse, friend, family member.

I would also recommend Redemption by Wilkerson and Redemption Groups. However, for the victim/survivor, I suggest reading Beauty for Ashes with a trusted friend, pastor, or counselor first. From my perspective, this would be a more gentle approach to such a painful process.

I am so excited that there is a growing number of safe, and solid biblical books addressing sexual assault! Our great God is the Soulologist who can heal wounds that can't be mended by anyone else. Thank You Lord for this work!

Beauty for Ashes Great Subtitle

Best book I've read on this extremely sad, but much needed topic. As a Biblical counselor, I've given it to both counselees and pastors. I love the way Kellemen talks about how David should have responded to Tamar--in a way that would have helped her replace ashes with beauty by assuring her of his love and reminding her that he saw her as spiritually pure, as did the King of Kings. The author beautifully depicts this scene as one taking place while David offers her a new royal garment. What a beautiful way of helping the violated overcome the shame that has been cast upon them!

About the Author
Graeme has been a practising medical herbalist for over 20 years, including two years working for the NHS. He is currently the Course Leader on the Herbal Medicine degree at the University of Central Lancashire and is an external clinical examiner for other herbal training programmes in the UK. He is a member of the Editorial Board of the Journal of Herbal Medicine and lectures internationally. Graeme lives in Lancashire, UK.
